Indications/Uses
Risponz 1/Risponz 2/Risponz 3 Schizophrenia & other psychoses; short-term treatment of acute manic or mixed episodes associated w/ bipolar disorder; treatment of irritability associated w/ autistic disorder. Risponz Schizophrenia & moderate to severe manic episodes w/ bipolar disorder. Short-term symptomatic treatment (up to 6 wk) of persistent aggression in conduct disorder in childn 5 yr & adolescent w/ sub-ave intellectual functioning or mental retardation.
Dosage/Direction for Use
Risponz 1/Risponz 2/Risponz 3 Initial dose: 2 mg on the 1st day, 4 mg on the 2nd day & 6 mg on the 3rd day. Dose adjustments may be needed in intervals of not <1 wk. Maintenance dose: 4-8 mg daily. Extrapyramidal symptoms >10 mg daily. Max: 16 mg daily. Risponz Adult Schizophrenia May be given once or bid. Initially 2 mg daily. May be increased to 4 mg on the 2nd day. Manic episodes in bipolar disorder Initially 2 mg once daily. Dose adjustment may be done in increments of 1 mg daily. Dose range: 1-6 mg daily. Persistent, aggression in patient w/ moderate to severe Alzheimer's dementia Initially 0.25 mg bid. May be adjusted in increments of 0.25 mg bid. Max duration: 6 wk. Childn & adolescent 5-18 yr Conduct disorder Subjects ≥50 kg Initially 0.5 mg once daily. May be adjusted in increments of 0.5 mg once daily, <50 kg Initially 0.25 mg once daily. May be adjusted in increments of 0.25 mg once daily. Elderly Initially 0.5 bid. May be adjusted w/ 0.5 mg increments to 1-2 mg bid.
Administration
May be taken with or without food.
Contraindications
Risponz 1/Risponz 2/Risponz 3 Known sensitivity to risperidone. Pregnancy & lactation. Childn <5 yr. Risponz Hypersensitivity. Childn <18 yr.
Special Precautions
Patients w/ renal or liver impairment, CV diseases & Parkinson's disease. May cause hypotension & wt gain. Elderly. Risponz 1/Risponz 2/Risponz 3 May impair mental alertness & ability to drive or operate machinery. Risponz Discontinue if tardive dyskinesia, NMS occur. Parkinson's disease & dementia w/ Lewy bodies. Not for patients w/ other types of dementia other than Alzheimer's. Patient w/ known CV disease (eg, heart failure, MI, conduction abnormalities, dehydration, hypovolemia, or cerebrovascular diseases); family history of QT prolongation, bradycardia, electrolyte distubrances (hypokalemia, hypomagnesemia); history of seizures or other conditions that lower seizure threshold. Patients w/ DM & w/ risk factors for developing DM. VTE. Preexisting hyperprolactinemia & in patients w/ possible prolactin dependent tumors. Concomitant use w/ furosemide; medicines known to prolong QT interval. Pregnancy & lactation. Childn & adolescent.
Adverse Reactions
Risponz 1/Risponz 2/Risponz 3 Insomnia, agitation, extrapyramidal disorder, anxiety headache & sedation. Somnolence, fatigue, dizziness, impaired concentration, constipation, dyspepsia. Nausea, vomiting, abdominal pain, wt gain, blurred vision, priapism, erectile, ejaculatory & orgastic dysfunction, urinary incontinence, rhinitis, rash. Risponz Anxiety, dizziness, dystonic reactions, extrapyramidal reactions, headache, NMS, tardive dyskinesia; abnormal t-waves w/ prolonged ventricular repolarization, arrhythmias, orthostatic hypertension, amenorrhea, galactorrhea, gynecomastia, sexual dysfunction; wt gain, constipation, nausea, cholestatic jaundice, agranulocytosis, overflow incontinence, priapism, urinary retention, altered temp regulation.
Drug Interactions
Increased risk of sedation w/ centrally-acting substances notably alcohol, opiates, antihistamines & benzodiazepines. Increased plasma conc w/ phenothiazines, TCAs, some β-blockers & fluoxetine. Decreased plasma conc w/ carbamazepine & other hepatic enzyme inducers. May antagonize levodopa & other dopamine agonists. Risponz Medicinal products known to prolong QT interval eg, class Ia antiarrhythmics (eg, quinidine, dysopiramide, procainamide), class III antiarrhythmics (eg, amiodarone, sotalol), TCAs (eg, amitriptyline), tetracyclic antidepressants (eg, maprotiline), some antihistaminics, other antipsychotics, some antimalarials (eg, mefloquine), medicines causing electrolyte imbalance, bradycardia, or those which inhibit the hepatic metabolism of risperidone. Hypotension w/ antihypertensive treatment. Increased plasma conc w/ verapamil. Increased bioavailability w/ cimetidine. May lead to additive active antipsychotic fraction exposure in concomitant use w/ paliperidone. Increased mortality in elderly w/ dementia w/ furosemide.
